Employee injured in fall down elevator shaft

Employee #1 was coming down from the roof of a building. He entered an interior hall from the bright sunlight and stepped through an opening to go down the stairs. Employee #1 fell 12 ft down an unguarded elevator shaft into the sump, sustaining injuries that required hospitalization.
