Employee injured when loaded cart overturns

Employee #1 had loaded panes of glass onto a material handling transport cart designed for this purpose. He was trying to move the cart to another location to pull inventory when it overturned. Employee #1 suffered a fractured ankle and a minor back laceration, for which he was treated and released. He had received specific instructions and training regarding the hazards of loading transport carts. Employee #1 had not distributed the load evenly, causing the cart to become unstable and to tip over.
