Investigation Summary
Employee #1 was standing at ground level, engaged in touch-up paint work on a cinder block wall. He was standing between an aerial lift basket and the wall when Employee #2 moved the basket, crushing Employee #1's head between the wall and the basket. He was killed. Employee #2, age 18, suffered from shock and distress, and was hospitalized shortly after the accident.
