Employee killed when crushed by falling stud wall

Employee #1, age 18, and two coworkers were trying to place on its anchor bolts a 2 by 6 veneer-laminated stud wall that was covered on one side by wafer board. Employee #1 was on the wall's west side and one coworker was on its east side. The other coworker was using a 2 by 6 board to move the bottom of the wall from the east to match up the holes drilled into the plate. Employee #1 tripped and the wall came down on top of him, crushing him against the concrete slab. He was killed. Employee #1 tripped on either an anchor bolt sticking out of the concrete or on a small, 3 in. rise in the concrete for an adjacent wall.
