Employee's Hand is Caught in Sprocket Gears

On November 21, 2003, Employee #1 was cleaning masa corn meal dough that fell between the dough rolls, and the gears and the frame of the machine, when his hands were caught in the sprocket gears. The sprocket gears were unguarded. A review of safety program revealed that employer the did not have an operational Lock-out/Tag-out Program for the machine operator(s). Employee #1 was hospitalized and treated for his injuries.
