Employee's arm fractured when caught in milling machine

At approximately 3:00 p.m. on November 17, 2003, Employee #1, a machine operator, was operating a Bridgeport Milling Machine, Series I and Serial Number J-88930, to mill rotating arms for airport security tables. The arms dimensions were 1 in. by 2 in. by 10.75 in. They were milled to a concave shape at both ends. The ends were cut by setting the arm horizontally on a vice and gradually pushing it against the mill's cutting bit with a pushing block. When one end is done, the arm is manually turned around to do the other end. Employee #1 grabbed the end which she just completed milling with her right hand while the mill was running. Her glove was caught by the rotating bit, wrenching her hand. The bit then caught her jacket's long sleeve and wrenched her upper arm. As a result, she sustained bone fractures on the right forearm which required surgery and hospitalization.
