Investigation Summary

Investigation Nr: 201503281
Event: 11/22/2002
Employee Suffers Thumb Amputation

At approximately 4:45 p.m. on November 22, 2002, Employee #1 was using a Dewalt Model DW708 sliding compound miter saw to crosscut three stacked pieces of 0.75-in. thick wheat board that measured 4.5 in. wide by 11.5 in. long. Employee #1 was holding the material with his left hand within the 6-in. danger zone of the saw blade. The saw in the closed position has an unguarded section of blade 1.75 in. tall at the front of the blade. The top left side adjustable saw fence was not properly adjusted to support the material. The fence was adjusted approximately 5 in. to 6 in. away from the saw blade. The lower fixed saw fence on the saw is 1.25 in. tall. Employee #1 attempted to cut the material to a length of 9.5 in. When the blade contacted the unsupported top board, the blade caught the material pulling the board behind the fence and Employee #1's left thumb into the unguarded area of the front blade. Employee #1 suffered the amputation of his left thumb and was hospitalized.
