Investigation Summary
At approximately 12:21 p.m. on June 26, 2002, Employee #1 was assisting a coworker repairing a roll-up door when the tension bar that his coworker was using slipped and hit Employee #1 in the neck. Employee #1 was immediately transported to the hospital where he was examined and released. Employee suffered minor bruising of the neck. Employee #1 stated that his neck was sore for a few days, and that he believed the tension bar his coworker was using was not large enough for the job.
