At approximately 11:00 a.m. on March 29, 2002, Employee #1 attempted to align the raised mast of a forklift truck by pulling it with his left hand. He lowered the load while his hand was exposed. The mast caught the hand crushing the fingers. He suffered a severe left hand crushing injury. Employee #1 was hospitalized for 5 days post reconstructive surgery.
