Investigation Summary
On January 21, 2002, Employee #1, a maintenance employee, attempted to adjust the slack on the "CC" conveyor chain. Employee #1 did not deenergize, lockout, or tagout the 120-volt control circuits, the hydraulic systems, and the 480-volt conveyor supply circuits. The employer did not develop, implement and utilize an energy control procedural steps for shutting down, isolating, blocking, and securing machines or equipment to control hazardous energy during maintenance and adjusting operations. Employee #1 was lying on and reaching through the energized conveyor "AH" to adjust the "CC" conveyor chain take-up. Employee #1 triggered the micro switch on the stop gate that opened the hydraulic racing valve serving the stack pushers, which engaged the stack pusher that fatally injured Employee #1.
