Investigation Summary
On September 9, 2003, Employee #1 and four coworkers were traveling in a 1995 Dodge Dakota extended cab pick-up-truck. Employee #1 was driving the vehicle with one coworker in the passenger seat and the remaining three in the back seat. For some reason, Employee #1 lost control of the car and drove into a reservoir that was approximately 4-feet deep. Employee #1 sustained a chest injury and was transported to the University Medical Center in Fresno, California, along with the coworker located in the front passenger seat. He sustained no injuries. The three employees riding in the back seat of the truck were unable to exit the truck's cab and drowned. They were pronounced dead due to asphyxia and were transported to the University Medical Center Morgue.
