Investigation Summary
On May 29, 2007, Employee #1 was repairing a conveyor system. He was lying on the conveyor belt and asked a coworker to jog the system. Neither employee was wearing fall protection. The conveyor belt activated, and Employee #1 fell 75 feet and was killed. The causal factors of this incident were a lack of fall protection and failure to lock out/tag out the conveyor system.
