Investigation Summary
On March 29, 2007, an employee was placing the roll on another stack of coil. The C-Hook, 1500 lbs, slipped out from the coil, traveling with a high velocity of speed. The C-hook struck the employee in the chest area and was knocked 20 feet across the warehouse. The employee sustained head, back, neck injuries. The employee was treated at the scene by emergency medical personnel, transported to the hospital, and remained there for 12 days.
