Employee Is Injured in Fall from Ladder

On August 16, 2004, an employee was conducting a roof and nail inspection on the second story roof of a new home under construction. The employee ascended to the first story roof via an aluminum extension ladder with a foreman and a laborer. The employee then ascended to the second story roof from the first story roof following the foreman, while the laborer held a second aluminum extension ladder, Werner Mode Number D11162. A 2 by 4 wood cleat was also nailed behind the feet of the ladder to keep it from slipping on the sloped roof. The employee completed the inspection of the second story roof in less than five minutes. The foreman then descended the ladder without incident. The employee then began to descend the extension ladder while the laborer held the ladder. When the ladder began to collapse, the employee fell off the ladder and struck the first story roof and then fell to ground level. The total fall distance of the employee was approximately 18 feet. The employee suffered head injuries and three fractured ribs. The ladder was inspected and found to be in good condition and would not collapse when tested. The employee was hospitalized and treated for his injuries.
