Employee injured in fall descending scaffold ladder

At approximately 1:20 p.m. on the day of the accident, Employee #1 was working from a scaffold inside a Flexicoker heater. He was descending the scaffold ladder, carrying an empty light bulb box under his right arm. and using his left hand to grab each rung as he went down. At a height of between 12 and 14 ft, he grabbed for a rung, missed it, and fell to the bottom of the vessel. Employee #1 sustained a concussion, for which he was hospitalized.
