Employee's finger is amputated when caught in conveyor

On August 26, 2003, Employee #1, a maintenance worker at Circle Food Products was assigned by his supervisor to clean and to oil the chains on the Tortilla Cooling Conveyor. The chains to this cooling conveyor were accessed by removing a sheet metal machine guard and were cleaned and oiled with the machine running. The chains were part of a chain and sprocket system. Employee #1 was told by the Maintenance Supervisor to oil the chains with the machine on and Employee #1 was not given any instructions on how to clean or oil the chains. Employee #1 was also not trained on lock out and tag out procedures. The Tortilla Cooling Conveyor was manufactured by Falcon Machinery Manufacturing Company. Employee #1 removed the machine guard while the cooling conveyor was running. Employee #1 oiled and applied WD-40 to the chain. Employee #1 then applied a shop rag over the moving chain and sprocket to clean the dusty and dirty chain. The rag was pulled in and pulled Employee #1's hand in with the rag. The chain and sprocket amputated Employee #1's right index finger. Employee #1 was working alone, so there were no witnesses. Employee #1 was hospitalized. This accident could have been prevented if Employee #1 turned off, locked out and tagged out the energy source of the Tortilla Cooling Tower.
