Investigation Summary
On January 31, 2003, Employee #1, of Capital Tile Co. Inc., had driven a Powered Industrial Truck, PIT, off the edge of a concrete sidewalk area and stuck the vehicle. The clearance under the PIT was 4 in. and the drop-off at the edge of the sidewalk edge was 6 in., causing the PIT to bottom-out. Attempts to free the PIT were unsuccessful until the supervisor arrived at the jobsite and decided to pull the PIT with his large 4 by 4 pickup truck. The supervisor directed Employee #1 to remount the PIT to steer and drive it up onto the sidewalk, as he pulled it with his pickup truck. A nylon web sling was attached to the trailer hitch of the pickup truck and then to the mast on the PIT, and the pull began. As the PIT was being dragged down the edge of the sidewalk, it started to turn sideways up onto the concrete surface and bounce along its left bottom edge. It then flipped over. As the PIT began to overturn, Employee #1 exited the seat and was crushed by the overhead guard. The employee was not wearing a driver restraint device that was installed on the equipment.
