EMPLOYEE KILLED IN FALL IN ELEVATOR SHAFT

On March 31, 1998, a hospital visitor dropped his driver's license down the second floor elevator shaft. Employee #1 and a coworker, hospital maintenance employees, ran the elevator to the top of the shaft and attempted to retrieve the driver's license. Employee #1 climbed inside of the hoistway on the first floor and had his coworker partially close the elevator doors keeping both hands between the doors in order to prevent them from closing all the way. Another person called the elevator to the first floor. The moving elevator caused Employee #1 to fall approximately 25 ft to the bottom of the elevator pit. Employee #1 was killed. The elevator was not locked out, and the employees were not provided any type of fall protection. The employer's lockout/tagout program requires employees to contact the elevator company to perform any maintenance or repairs on elevators.
