Forklift driver killed when crushed by steel beams

Employee #1 was operating a Yale stand-up rider forklift to move some metal baskets used to carry materials for the making of jeans. He placed the rack on the forks, drove it into the storage receiving area, and sat it down on the floor. He then began to back up without looking behind to observe his path of travel. Employee #1 backed into a metal column that was supporting the overhead storage on the northwest side of the storage and receiving area. When the forklift struck the column, it broke loose from the concrete where it was bolted and from the upper portion of the storage area. This caused the steel I-beams to collapse on the forklift. Employee #1 was killed when he was crushed against the console of the forklift.
