Employee crushed by stacked steel

Two employees were tasked to restack steel in the receiving area. The steel was stacked at various heights with some stacks 10 ft high. Most stacks were bundled, but some steel on top of the taller stacks remained loose. Employee #1 was in between two stacks of steel, using a chain saw to cut 4-ft by 4-ft steel that was protruding from a bundled stack in order to place stacks closer together. The other employee was operating a pendant control to the overhead crane that was used to move the bundled stacks. A stack of steel fell onto Employee #1, crushing and killing him.
