Investigation Summary

Investigation Nr: 201320124
Event: 10/14/1998
Employee died of a heart attack

The general manager and a coworker greeted each other at the beginning of a workday and started to set up five Kasto saws used to cut metal plates into bar stock. The facility was large, and the employees could not see each other as they started at opposite ends of the plant. After he completed setting up his third saw, the coworker became concerned about general manager who should have finished. The coworker found the general manager unconscious at the first saw on his end. The general manager, who had been wearing gloves with his sleeves rolled down, had collapsed with his torso resting on top of the table with his feet extending onto the control station platform. There was a pool of blood from a head laceration. His lips were cyanotic, and his skin was cool to the touch. The coworker administered cardio-pulmonary resuscitation. Emergency medical services transported the unconscious employee to the hospital, where he was pronounced dead on arrival. The autopsy indicated that the head wound was not the cause of death. Thorough examination of the electric equipment near the saw did not discover any electric shock hazards. Due to the lack of physical evidence to conclude otherwise, the pathologist ruled the cause of death as sudden cardiac death associated with hypertrophic cardiomyopathy.
