Investigation Summary

Investigation Nr: 201180627
Event: 04/02/2001
Employee injured when fork lift ran over foot

On April 2, 2001, Employee #1 was working for the San Francisco Food Bank as a truck driver. He had driven a load of food products to the dock for unloading. A pallet of food was unloaded by a coworker with a fork lift and was brought to a scale for weighing. Employee #1 read the weight from the scale to the fork lift operator, who recorded the information on a log. Employee #1 was standing alongside the fork lift. At approximately 10:20 a.m., the fork lift backed-up, and in the process, ran over Employee #1's left foot. Emergency services were called and Employee #1 was taken to San Francisco General Hospital. He was treated for a fractured toe and released the same day.
