Employee Is Injured in Fall from Forks of Lift

On August 25, 2005, an employee was working in the packaging department where milled rice is stored in one metric ton totes and these are in turn stacked up to three high. When ready to ship, the stacks are broken down via a forklift whose operator drives the forks through loops located in each corner of the tote. The employee assisted the forklift operator by standing on top of the tote to guide the forks through the loops. On the day of the accident, the employee was standing on a tote in preparation for the forklift operator to elevate him to the top of a stack of totes. Before they could move very high, two totes stacked behind the employee fell over, striking the mast of the forklift and pushing the employee into the forklift. The Williams Fire Department responded, and the employee was transported to the Colusa Regional Medical Center in Colusa. The employee sustained a laceration to his forehead that required some 24 stitches to close the contusion to his left hand. The employee was released several hours after being admitted.
