Investigation Summary

Investigation Nr: 201177391
Event: 05/26/2005
Employee Climbing Wall Falls and Sustains Fractures

The accident took place in the wall climbing area of a recreation facility. The climbing wall is approximately 30-ft high, and has three climbing patterns (advanced, moderate, beginner). Users of the wall are given a harness and are secured to a belay (safety device) throughout their climb. A customer (young child) was attempting to climb the wall and was having problems. Employee #1 was working at the climbing wall disconnected the customer from the belay (fall protection) and climbed the wall to demonstrate to the customer on proper climbing techniques. Employee #1 climbed the beginner climb, jumped to hit the bell at the top (approximately 30 ft above ground level) and fell straight to the ground. Employee #1 failed to secure himself to the fall protection (belay and ropes) when he began his climb. Employee #1 broke his ankle and his leg. Employee #1 was hospitalized for three days.
