Investigation Summary

Investigation Nr: 201177367
Event: 02/10/2005
Employee Is Injured When Struck on Head by Flying Object

On February 10, 2005, an employee was assisting in the production and packaging of roof panels inside a building. After each panel was made, it was placed on several rolling racks and when the desired bundle of panels was assembled it was secured via a 2-in by 4-in. frame. A single metal band was precut and wrapped around the outside of the wooden frame and suing a manual tensioner, the band was pulled tight. As the employee was trying to remove a gap in the wooden frame, he placed considerable pressure on the tensioner. It appears the band broke, resulting in the unit striking him in the head causing him to fall backward on the floor. He was transported to a nearby medical clinic by a coworker but the clinic staff decided to call the West Sacramento Fire Department, who transported the employee to a Kaiser hospital. The employee sustained a slight concussion with a small puncture wound to the right side of the head along with a general thoracic and lumbar strain. He was released from the hospital the same day.
