Event: 11/23/2004
Employee Falls Through Roof and Is Hospitalized

At approximately 10:40 p.m. on November 23, 2004, Employee #1 was working with two other employees of Pacific Supply Company delivering roofing materials to a jobsite. The three employees would load a cart with materials, roll it to a designated work area, unload the cart, and return to the drop off area for more material. They had just unloaded the material to a work area on the roof and were returning to the material drop off point. Two employees left Employee #1 to pull the cart back to the drop off point. Employee #1 was pulling the cart and walking backward when he stepped through the roof. An employee of the roofing contractor had removed a piece of roof sheeting leaving only the insulation. Employee #1, unaware that he was approaching this hazard, stepped onto the insulation and fell 20.5 feet to the floor below, sustaining multiple leg fractures, for which he was hospitalized.
