Employee's Leg Is Amputated in Fall from Utility Pole

On January 24, 2005, Employee #1, a lineman, was in a field of training poles, approximately 38 ft tall. He was taking his final test of the week long climbing school course. He completed his job of installing cross members at the top of the pole. He dropped his supply rope, unhooked his belt and started to descend. He moved approximately 4 ft, and his right gaffe cut out of the pole, causing him to lose his hold and fall approximately 30 ft. He was hospitalized, and his right leg was amputated.
