Employee is injured in fall from roof when ladder slips

On October 22, 2004, Employee #1, a warranty technician working for Dunmore Homes, was performing a service call at a residence. He set one end of a portable ladder, Serial Number 0311-22, on the driveway and rested the upper part of the ladder against the edge of the roof over the garage. He carried four tiles up the ladder and set them on the roof. He then retrieved a caulking gun and set that on the roof as well. He descended the ladder again for some reason and then climbed back up the ladder to get onto the roof. While attempting to get onto the roof, he shifted his weight to place a leg on the roof and the ladder slipped out from beneath him. He fell approximately 11 feet 8 inches to the concrete driveway and suffered serious injuries to both wrists, for which he was hospitalized. Further investigation revealed that the ladder was not tied, blocked, or otherwise secured. Additionally, a second employee was not dispatched to help Employee #1 because it was company policy to send only one warranty technician to work on single-story homes.
