Investigation Summary
Accident occurred at about 10:00 am, 9/3/2009 and was reported to DOSH by employer on 8/3/2010. EE#1 was installing sheet of plywood on the roof. Contact was made to exposed electrical line. EE#1 fell from the roof which is 24 feet to the ground. EE#1 got the job from the employer to replace damaged wood and install plywood for the roof. EE#1 worked for the employer couple times a year for the past ten years. EE#1 had EE#2 as an assistant working with him at the time of the accident. The working place is a single family house located at 5108 Valley Ridge Ave., Los Angeles. The project is in a remodeling and fixing nature. The roof EE #1 and #2 were working on is part of the house where underneath a two-car attached garage being on street level and a living space being above the garage. On 9/3/2009, at north-east corner of the roof, EE#1 was kneeling and chalk lining to mark the nailing position on the plywood. He accidentally contacted the electrical wire and fell from the roof.
