Employee Fatally Falls from 30 Foot Wall

At approximately 12:08 pm, on August 2, 2011, Employee #1 was working with a crew to install a new poured-in-place concrete methane gas compression building. Forms for the walls were being erected; with the one involved in the accident was thirty feet tall and 8 feet wide. It was to be attached to a shorter concrete wall 54 inches wide, and braced with two steel panel braces. It sat on two 6 by 6 in. leveling jacks spaced 34 inches apart. This spacing meant that the wall, which weighed 3000 pounds, sat on the center of the panel which left the right 48 inches of it unsupported. Employee #1 was standing on the top left side of the panels with his fall protection tied to the panel. He was plumbing and aligning the panel. As he worked the panel fell to his right and rotated 90 degrees. Employee #1 died due to a concussion.
