Investigation Summary
At approximately 11:30 a.m. on June 11, 2010 Employee #1 was attempting to widen the trench footing in a four and a half foot deep excavation. As Employee #1 attempted to cut slots in the base of the trench sides, a block of soil fell on him, causing crushing injures requiring twelve days hospitalization and surgery.
