Investigation Summary
At approximately 10:30 a.m. on August 4, 2009, Employee #1 was working from a 10-ft long by 5-ft wide by 10-ft high metal rolling scaffold at a new single family residence project site. The supervisor instructed Employee #1 and a coworker to install metal stud framing in the powder room located on the first floor. Employee #1 stated during an interview that the coworker helped him erect the rolling scaffold after stocking the area, finishing laying the track down and getting material ready for the task. Employee #1 and the coworker had assembled the frame and cross braces of the first level successfully prior to the accident. Employee #1 climbed the frame of the scaffold to erect the second level of the scaffold. Employee #1 had both feet on the scaffold frame when the coworker handed him the brace. Employee #1 was approximately 4-ft to 6-ft from ground level while he stood on the scaffold frame. Employee #1 attempted to install the brace on the frame, lost his footing, let go of the brace and fell to the ground and landed on both feet. Employee #1 sustained an open fracture of his distal tibia and fibula (lower left leg). Employee #1 was hospitalized.
