Employee is injured in fall from forklift basket

At approximately 1:30 p.m. on February 2, 2004, Employee #1 was in a metal basket suspended approximately 14 feet above the ground by a forklift. He was troubleshooting an overhead crane that was not functioning properly. He shifted his weight to the end of the basket to open the door on the overhead crane electrical box. He then felt the basket move and realized that the forks of the forklift were not in the fork slots beneath the metal basket. He fell approximately 14 feet to the ground and suffered several fractures. He was transported to Arrowhead Regional Medical Center where he was hospitalized for 17 days.
