Employee's ankle fractured by falling wood frame

At approximately 7:50 a.m. on September 15, 2003, Employee #1 was on an unshingled roof collecting extra plywood sheets so a coworker driving a forklift could pick them up. When the lift truck arrived, it was carrying the wood frame for a fireplace chimney. The coworker raised the 30 in. by 32 in. by 42 in. fireplace stack to the top of the roof. He told Employee #1 to push on the approximately 80 lb frame to unload it from the forklift. Employee #1 was holding onto the frame and was standing in front of it, approximately 3 ft from the eave of the roof, when he felt the load start to slide down. As he tried to grab it, Employee #1 was pushed down by the moving stack and he fell with it approximately 9 ft 11 in. to the ground. The fireplace frame landed on his right ankle, causing a compound fracture. Employee #1 underwent surgery and was hospitalized for three days.
