Investigation Summary
At approximately 8:30 p.m. on August 11, 2003, Employee #1 was in the forming department working on the Sheridan-Gray Stretch Press taking off the 12-in. steel head that secures the material that is being stretch. The steel head weighed approximately 800 pounds. Employee #1 used a pipe wrench to loosen the steel head, and, once loosened, he started turning the steel head in a circular motion until the steel head spun to the end of the cylinder. The head was not intended to come off the cylinder, but Employee #1 miscalculated the number of spins the steel head had taken and the steel head fell off the cylinder hitting his left hand. Employee #1 sustained crushing injuries to three fingers on his left hand. Employee #1 was hospitalized.
