Investigation Summary
At approximately 1:06 p.m. on August 4, 2003, Employee #1 went up on the roof to remove some pieces of plywood that were placed on the roof by unauthorized unknown skateboarders. A piece of plywood had nails in it that stuck it to the roof. Employee #1 yanked the board and it broke loose. Employee #1 fell a distance of 9-ft-4-in. off the roof. Employee #1 suffered seven broken ribs and a punctured lung. Employee #1 was hospitalized.
