Investigation Summary
At approximately 4:00 p.m. on June 20, 2003, Employee #1 was adding caustic soda to a boil-out vat used to clean oily and dirty radiators. The employer was aware that the solution in the vat was hot and more active and he then warned Employee #1 to be careful. Since it was drizzling, the employer suggested putting about six cups of caustic soda in a bucket to keep the bag of caustic soda dry. Employee #1 added the whole bucket into the vat at one time. This caused the hot solution to erupt from the chemical reaction and splashed the hot liquid and chemical on the front side of Employee #1's body. The employer was approximately 10 ft from Employee #1 at the time of the accident and turned the water faucet on and hosed Employee #1 with water. Employee #1 sustained second-degree and third-degree burns on of his body and legs and was hospitalized.
