Investigation Summary

Investigation Nr: 201157070
Event: 03/12/2003
Employee Amputates Finger

At approximately 11:23 a.m. on March 12, 2003, Employee #1 and another employee were clearing the jammed chute of the grinder in the Finishing Pig Mill Second Grinding Unit. Just before the accident, he and the operator were trying to open the door on the chute (door opens up downward because it is hinged at the bottom). The employee was holding the door on one side and the operator was holding the door on the other side. Employee #1 took the screw out on his side with his left hand. The door came down and his right index finger was caught between the door and the metal edge of the lower part of the chute, amputating his finger. The door had a lot of pressure due to the clay inside the chute. Employee #1 was hospitalized.
