Investigation Summary

Investigation Nr: 201157088
Event: 02/14/2003
Employee Suffers Amputation of Toe

At approximately 8:00 p.m. on February 14, 2003, Employee #1 and three coworkers were off-loading a cargo plane (Tail Number N-328, UPS 2968) inside the cargo area. Just before the accident, Employee #1 was pulling pallet Number 41950 that was loaded with fruit boxes weighing approximately 5650 pounds. Employee #1 was walking on the side of the load. There was another employee pulling on the other side and two employees were pushing the load from the back. Somehow Employee #1's right foot got caught between the butterfly-type locking device on the floor and the pallet and his foot was crushed. Employee #1's right fourth toe had to be amputated. Employee #1 was hospitalized.
