Investigation Summary
At approximately 11:30 a.m. on January 6, 2003, Employee #1 was removing a toe board from a roof eave when he lost his balance and fell from the 8 ft step ladder that he was using to gain access to the roof eave Employee #1 sustained a fracture of the L-2 vertebrae of his lower back as a result of the fall and then landing on a steel clip that was attached to his tool belt. Employee #1 fell approximately 5 ft as he was standing on the fifth rung of the aluminum ladder. The roof eave measured 7 ft 6 in. in height. Employee #1's coworker was holding and stabilizing the ladder when the fall occurred. Employee #1 was hospitalized for his injuries.
