Employee Sufferes Amputation of Finger Tip

At approximately 11:30 p.m. on January 22, 2003, Employee #1 was attempting to adjust the path of the polyethyelene sheeting that was not feeding properly through an industrial bagger and barcode machine (Number 5 Filmstar Bagger, Model Number 812B, and Serial Number 300987.) The accident occurred when the heat sealing jaws of the bagger inadvertently closed while Employee #1's right hand was in the point of operation which resulted in the amputation of the tip of his right middle finger. Employee #1 was not hospitalized.
