Event: 10/28/2002

On October 28, 2002, Employee #1 was manually stacking empty 55-gallon plastic drums in a warehouse. Each stack of drums contained four drums with numbers 1, 2, and 3 placed in ascending order and the number 4 drum was placed on top of the number 3 drum. For some reason, while tossing the number 4 drum on top, Employee #1 had his left ring finger resting where the second and third drums meet. The fourth drum fell, striking Employee #1's left hand.
