Employee cuts three fingers while using a table saw

At approximately 6:45 a.m. on June 11, 2002, an employee was in the Mill Department operating a table saw, Powermatic Model Number 72A, Serial Number 9972017. He was cutting a 10-in. by 10-in. piece of wood into 2-in.-wide strips. He had cut approximately 12 pieces. While cutting the next piece, his left hand came into contact with the saw blade. He received lacerations and fractures to his left thumb, index and middle fingers. The employee was taken to Desert Regional Medical Center where he was hospitalized and had surgery.
