Employee injured when struck by forklift

At approximately 5:07 p.m. on May 22, 2002, Employee #1, a full time employee of Exel Logistics Inc., was working in an 825,000 square foot warehouse and trucking facility. There are approximately 1.6 million tires of all types and uses in storage and distributed out of this facility. Employee #1 was standing on the warehouse floor inventorying a load of tires when he was struck by an electric Yale industrial truck (forklift), Model Number 391A with a 4000-pound capacity that was moving in reverse. The employee sustained an amputation of his left foot/leg at the ankle. He was transported by helicopter to the Loma Linda University Medical Center where he was treated and hospitalized for 4 days.
