Employee's Finger Is Crushed in Rollers, Later Is Amputated

At approximately 1:40 a.m. on June 26, 2006, an employee was cleaning a refiner machine using a Scotch-Brite pad. The machine was not locked out or deenergized. The safety grid was not installed and the interlock limit switch for the grid was not functional, as the unit was able to operate under power without the guard in place. Employee #1's work glove got caught between the rolls, and his fingers were pulled into the in-running nip point, crushing them. He was hospitalized and his left index fingertip was amputated.
