Investigation Summary
At about 12:30 a.m. on May 31, 2005, Employee #1 was unloading a mail bag container. At the time of the incident, he was exiting a lowered container that had loose plastic on the floor. The plastic caused him to slip and fall head first down between a pit's perimeter wall and the mail bag container. The opening was approximately 45 in. deep. He suffered a fractured hip and was hospitalized.
