Investigation Summary

Investigation Nr: 201125432
Event: 01/19/2005
Employee Is Injured When Pinned Between Two Vehicles

At about 1:00 p.m. on January 19, 2005, Employee #1, of Gimbal Brothers, was driving a forklift to help another person, not an employee, unload the flatbed truck. Employee #1 dismounted from the forklift to help the other man straighten a load on the flatbed so that the load's pallet would align with the forks of the lift truck. As Employee #1 was making this adjustment, the forklift rolled toward him and pinned him between the two vehicles, seriously injuring him. He was hospitalized.
