Employee is injured after falling from ladder

On November 3, 2003, Employee #1, a roofer along with an apprentice, was cleaning the roof of debris. Employee #1, who was acting as a leadman, set up his ladder on a pickup truck bed to gain height but did not lash or secure the ladder in any manner. As Employee #1 was going down, the foot of the ladder slipped and the ladder shifted. Employee #1 lost his balance and fell into asphalt concrete. He suffered fractures to the left foot, right knee, left thumb and left shoulder. He was hospitalized.
