Investigation Summary
On July 27, 2003, Employee #1 was operating a Winkler Stringline Proofer at a bakery when she noticed a piece of dough displaced from its moving trough. As she reached into the machine to remove the dough, her hand and arm became caught in a powered chain and sprocket drive. Employee #1 was hospitalized for treatment and surgery.
