Employee 's arm caught beneath forklift

At approximately 4:30 p.m. on June 24, 2003, Employee #1 was working alone in a fork lift truck unloading a batch of rebar from a truck. As he backed the lift truck down a steep driveway, with the forks in a high position, the fork lift tipped over backwards. Employee #1's arm got caught beneath the protective cage. He was seriously injured requiring extensive treatment in hospital.
